Vigorous Intensity Exercise
Dr. Patrick emphasizes that vigorous intensity exercise is the 'biggest anti-aging blockbuster' if it could be put into a pill. It's defined by the inability to hold a conversation or maintaining a heart rate at 80% or more of maximum. Cardiorespiratory fitness, measured by VO2 max, is a critical biomarker for healthspan and lifespan, with improvements directly correlating to increased life expectancy (e.g., 45 days per 1 ml/kg/min increase). A study in JAMA showed no upper limit to benefits, with elite fitness levels (top 2%) having an 80% lower all-cause mortality than low fitness, and even the top 0.25% having 20% lower mortality than those with 'high' fitness. Crucially, low cardiorespiratory fitness was a better predictor of mortality than type 2 diabetes or smoking, highlighting the sedentary lifestyle as a 'disease.' While moderate exercise is beneficial, vigorous intensity, especially HIIT, is necessary for many 'non-responders' to improve VO2 max. This is because greater stress leads to greater adaptation, enhancing oxygen transport and utilization throughout the body, including the brain and muscles.
Vigorous exercise forces the body to produce energy quicker than mitochondria can keep up, leading to anaerobic metabolism and lactate production. Lactate, once thought a waste product, is a crucial signaling molecule. It's transported to the brain, increasing Brain-Derived Neurotrophic Factor (BDNF) for neuron growth, learning, and neuroplasticity. It also boosts neurotransmitters like serotonin (for mood, impulse control) and norepinephrine (for focus, attention). The intense stress also drives greater physiological adaptation in the cardiovascular system.
I do 10 minutes of exercise like before almost any talk I give, anything anything I'm doing that requires my brain to be working. 10 minutes, everybody has 10 minutes, right? And it's amazing how you feel after that.
If you could pill up what vigorous intensity exercise does, it would be the biggest anti-aging blockbuster out there.
